Which application, which version, and which operating system. What you did, what you expected, and what happened instead. If something looks wrong on screen, a screenshot usually saves a round trip.

Send only what is relevant to the problem. We would rather not receive personal information we do not need in order to help.

Each application links its own support destination from its store listing, and that is the fastest route once an application is installed. Where a listing points here, this address is the right one.
